Waiver of Liability -- if your student-athlete registered with another organization, then you need this form!
Coach's Background Check -- mandatory for all new MSHL coaches (and anyone who has not yet been checked!). It is MSHL Policy that anyone who interacts with minor children or goes into the locker room MUST be screened.
Non USA Hockey game(s) approval -- if you play a non-USA Hockey registered team (for example National Federation High School or Prep school) you must file this form with the District Registrar before you play. Submit one form with a list of all your non-USA Hockey games. The District Registrar is: Mark Bodner
